📊 Track and Visualize Assessment Scores Over Time
We're adding score tracking and visual trends for clinical assessments like the PHQ-9 and GAD-7 so you can see at a glance whether a client is improving, plateauing, or getting worse.
Right now, if you want to compare a client's assessment scores across sessions, you have to open each submitted form individually and do the math yourself. With this update, scored measures from our template library will include automatic severity interpretation (e.g., Minimal, Moderate, Moderately Severe) and plot scores on a trend line over time. You'll see a clear graph with color-coded severity zones directly in the client's chart. We'll also retroactively calculate scores for assessments clients have already completed, so your historical data shows up on day one.

This gives you an objective view of treatment progress in under five seconds, makes it easier to justify continued care to payers, and helps you catch concerning changes early instead of relying on memory or manual comparisons.

I have struggled to find a really good measurement tool. I do know Joint Commission and CARF both allow for the PHQ9 to used as a measured outcome. So this would be fantastic.

Visual tracking and automatic assessment?
Hello, I use assessments (some unusual ones, like the Massachusetts General Hospital (MGH) Hair Pulling Scale). Would be great if there could be a feature not only for automatic scoring beyond the common assessments such as PHQ-9/GAD-7, but also quick visual graphing for therapists (and clients?) to see how the scoring results shift from week-to-week.

Hi there! Just once, but it does have to be within 48 hours to either side of a session so that we can be confident that the score is linked to the client’s functioning as reflected in that particular session.
